Job Duties/Responsibilities: • Review and analyze incoming patient data, including vital signs, symptoms, and other relevant health indicators • Identify and address any critical or out-of-range readings promptly and escalate as necessary according to established protocols • Monitor patients' vital signs and health data remotely using specialized monitoring equipment and software • Communicate with patients via phone to discuss their health status and provide guidance as needed • Explain medical terminology and procedures in an understandable manner, ensuring patients are informed and engaged in their healthcare management • Address patient concerns and questions promptly, providing reassurance and guidance where necessary • Manage a significant volume of patient calls within established timelines • Work towards achieving personal and team performance metrics • Help foster a collaborative and positive work environment • Maintain the Nsight culture of positivity • Other cross-training LVN duties as assigned Requirements: • Active Compact LVN/LPN • Passionate about patient care, possessing the ability to relate with empathy and compassion.
