About the position The Nutrition Educator will provide telephonic nutrition education to members and patients with chronic conditions, guiding them in selecting, preparing, and consuming foods that are appropriate for their specific health needs. This role is crucial as it supports individuals participating in a chronic care meals program, which is designed to assist those managing conditions such as diabetes, heart disease, and kidney disease. The educator will adopt a member-centric approach, collaborating closely with patients to identify and address their unique nutrition-related concerns and topics that matter most to them. Additionally, the Nutrition Educator will facilitate connections between members and other nutrition resources available through the health plan, system, or community, ensuring comprehensive support for their dietary needs. In this remote position, the Nutrition Educator will conduct telephonic sessions tailored to each member's enrollment in various programs, documenting goals and care plans from these educational interactions. Responsibilities also include scheduling follow-up calls, developing educational materials, and identifying community resources that can further assist clients. Continuous professional development is essential, and the educator is expected to stay updated on the latest nutrition knowledge through educational opportunities, professional publications, and networking within professional organizations. Other nutrition-related tasks may be assigned as needed, contributing to the overall mission of promoting health and well-being among members. Candidates must be licensed in either the District of Columbia or Kentucky, and the role requires a Bachelor of Science in Nutritional Science or Dietetics, along with certification as a Registered Dietitian. A minimum of two years of experience in providing nutrition education or counseling is essential, along with excellent communication skills and strong organizational abilities. The position demands a high level of independence and responsibility, as the educator will be making clinical decisions regarding the appropriateness of nutrition education for each member or patient. Responsibilities • Provide telephonic nutrition education sessions per member/patient; number of sessions offered varies based on the program the member/patient is enrolled in. , • Document goals and care plan from education sessions in the platform. , • Schedule follow-up calls with each member/patient. , • Develop nutrition education handouts and tools as needed. , • Identify community resources for clients as needed. , • Maintain and update professional knowledge by participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations. , • Perform other nutrition-related tasks as needed. Requirements • Bachelor of Science in Nutritional Science or Dietetics. , • Certification as a Registered Dietitian by the Commission on Dietetic Registration. , • Active licensure in District of Columbia or Maryland. , • 2+ years' experience providing nutrition education or counseling. , • Excellent oral and written communication skills. , • Strong organizational and interpersonal skills. , • Ability to make clinical decisions regarding the appropriateness of nutrition education for each member/patient. Nice-to-haves • Active licensure in Kentucky. , • Previous experience providing telephonic nutritional education or counseling. , • Training in health coaching, motivational interviewing, and intuitive/mindful eating framework.
