A Licensed Practical Nurse (LPN) provides skilled nursing care to clients as prescribed by the physician and in compliance with regulations as established by the State Board of Nursing, procedures of Cymatex Consults LLC, and the patients. ? The Licensed Practical Nurse (LPN) works under the supervision of the Registered Nurse Supervisor in the planning and delivery of client care. ? Assignment of the LPN to patients shall reflect the highest standards of nursing practice, shall be proactive and reactive to client needs, and shall be provided in a manner that promotes quality, continuity, and safety of patient care. The licensed RN may be assigned to give nursing care that requires the skill and/or judgment of a Registered Nurse. COVID-19 vaccination Card or letter of exemption LPN Required Skills ? Must have verifiable ability to perform relevant nursing skills: 1. Pediatric (as it pertains to assignment) 2. Adult (as it pertains to assignment) ? The contractor must have at least one year of clinical experience, which includes pediatric direct patient care within the last two years. ? Has not been convicted of, received probation before judgment, or entered a plea of nolo contendere to a felony or any crime involving moral turpitude or theft, or has any other criminal history that indicates a behavior that is potentially harmful to agency clients. ? Current malpractice insurance certificate. ? Ability to meet the physical requirements of the position. LPN Job Descriptions ? Provides nursing care to clients in line with the nursing process. ? Re-evaluate the short and long-term goals for the client. ? Assessment of client ? Implementation of Plan of Care and Treatment Goals ? Work with family and/or primary caregivers to accomplish treatment goals. ? Educate and train the family regarding the care of the client. ? Implement physician order as per protocol to include: a. Medication Administration includes oral route, subcutaneous, intramuscular, intra-dermal, and intravenous as ordered by the physician. ? Document in progress notes of both clinical and nursing skills, observations, and interventions. b. Understand the equipment used by the client to include O2 and other durable medical equipment as the client?s condition and diagnosis dictate. ? Can use antiseptic and other cleaning solutions for equipment disinfectants. ? Can document the equipment maintenance log, the working order, and daily cleaning of equipment. c. Feeding d. Understands and follows safety practices and emergency management. e. Other required nursing care ? Routine maintenance of the MARs and ADL (Activities of Daily Living) ? Communicates through the communication log to the family and other nurses involved in client care. ? Transcribe and follow physician orders. ? Maintain the client?s safety, comfort, cleanliness, and well-being by being involved in the client?s activities of daily living. ? Consult the physician regarding client problems, document the physician?s response, and evaluate the client?s response to treatment. ? Explain the treatment plan, its rationale, expected outcome, and prognosis to the family and other nurses involved in the client?s care. ? Perform other duties as assigned by the RN supervisor or RN designee. Available shifts and locations ?
